The following documentation will be required to finalize your child’s enrollment in the Bedford Jr. Bulldogs program. We understand that providing this documentation can be time consuming. It is important to be clear that these are mandatory and your child will not be eligible to play without all of the requirements being met. These forms will be due July 15, 2010.

2010 Registration Form – Parent/guardian should thoroughly read and complete both sides of this form. This form must be signed by both the parent/guardian and the player. Click here to open the form.

2010 Medical Form – Any standardized child health form for sports, camp, etc. is acceptable, provided that it is dated in 2010.

Proof of Age – (For players new to the Jr. Bulldogs program only.) Photocopies of birth certificates or current (non-expired) passports are acceptable forms of proof of age. The birth certificate must be a copy from the city, town, or state the child was born; it must be signed by the local town/city registrar or clerk and must carry a State Seal. Foreign birth certificates must be translated to English and must be notarized. Court documents for name change and/or adoption are required paperwork.

Report Card – A copy of the child’s current year school report card is required and must show the full 2009-2010 school year, all trimester/quarter grades, name of school and child’s name. This must be shrunk to fit on an 8 ½ x 11 sheet of paper.
